Please call our main office prior to completing the intake forms in order to confirm your insurance plan is accepted & to ensure we have current availability for scheduling.

You will receive a confirmation email once you submit your completed forms. If you don't receive this, you may need to recomplete the form.
If needed, you may complete a paper copy of the intake forms in-office prior to your appointment. In order to not interfere with your appointment, we ask that you arrive 20 minutes early to do so.

Release of Information
You may complete a Release of Information (ROI) HERE if you'd like to coordinate services with Aspen Ridge Counseling. You may take the completed form to an Aspen Ridge office, fax or e-mail it

On-File Credit Card Form
We offer an on-file credit card option, to be charged automatically after each appointment for your copay/deductible/coinsurance amounts. If you would like to opt-in for automatic payments, please contact our office by email or phone call to request the authorization form.
