You will receive a confirmation email once you submit your completed forms. If you don't receive this, you may need to recomplete the form. Contact our office with any issues completing the forms.

You may complete a Release of Information (ROI) if you'd like to coordinate services with Aspen Ridge Counseling.  You may take the completed form to an Aspen Ridge office, fax or e-mail it.

We also offer an option to store a credit card on file. This can be used to automatically charge your regular owed amount after a kept appointment. If you'd like to put your card on file, please call or email our main office at

If needed, you may complete a paper copy of the intake forms in-office prior to your appointment. In order to not interfere with your appointment, we ask that you arrive at least 20 minutes early to do so.
