Please call our main office prior to completing the intake forms to confirm your insurance plan is accepted & to ensure we have current availability for scheduling.

You will receive a confirmation email once you submit your completed paperwork. If you don't receive this, you may need to recomplete the form. Contact our office with any issues completing or submitting these forms.

You may complete a Release of Information (ROI) if you'd like to coordinate services with Aspen Ridge Counseling.  You may take the completed form to an Aspen Ridge office,

fax or email it.

You may complete a paper copy of the intake forms in-office prior to your appointment if you're unable to complete the digital version. In order to not interfere with your appointment, we ask that you arrive at least 20 minutes early to do so.

Please ensure you keep your contact and insurance information up to date with our office. If your insurance changes, please verify we are in-network with our office prior to continuing appointments.

If you do not have Medicaid coverage or in-network commercial insurance, and believe you may qualify for financial assistance, Aspen Ridge is proud to offer a Sliding Scale Fee based on household income and family size. To be considered for this program, please complete the application below and provide the requested supporting documentation.

If you are covered by Medicaid, please contact our office as you may be able to receive services at no out of pocket cost. If you need assistance applying for Medicaid, our office has resources that can be provided to you to do so.
