Complete our online Intake Forms prior to your appointment by using the link below. You will receive a confirmation email once you submit your completed paperwork. If you don't receive this confirmation, you may need to recomplete the paperwork. Contact our office with any issues completing or submitting these forms.

Print Intake Forms

You may complete a paper copy of the intake forms in-office prior to your appointment if you're unable to complete the digital version. In order to not interfere with your appointment, we ask that you arrive at least 20 minutes early to do so.

You may also use the links below to print a copy to complete ahead of time and bring to your appointment.


Release of Information


If you would like to coordinate services with Aspen Ridge, or authorize us to communicate with a third party, you may complete a release of information (ROI). The completed form can be taken to an Aspen Ridge office, faxed, or emailed.

A digital version is also available and can be requested by calling our main office.

Please ensure you keep your contact and insurance information up to date with our office. If your insurance changes, please verify with our office that your therapist is in-network prior to continuing appointments.
